It’s easy! All you need to do is contact your Human Resources department for a form or online link. If you’d rather, please contact Lifeworks at (giving@lifeworks.org or 651-365-3719) and we’ll do the checking for you! Once the form is completed, Lifeworks receives the matching dollars.
