CFSS replaces the Personal Care Assistant (PCA) and the Consumer Support Grant (CSG) programs beginning October 1, 2024. What is Community First Services and Supports?
Community First Services and Supports (CFSS) is a self-directed service that provides individuals with more control over their care. With CFSS, you can choose and manage your own caregivers, including spouses and parents of minor participants. You’ll also be able to purchase goods and services that help reduce the need for human assistance, like home modifications or assistive technology.

How is CFSS different from PCA and CSG?
● Expanded Caregiver Options: Unlike PCA, CFSS allows family members, including spouses and parents of minors, to be paid caregivers.
● Increased Flexibility: CFSS participants can purchase goods and services, such as home modifications, to reduce reliance on human assistance.
● Person-Centered Planning: CFSS introduces mandatory consultation services to help participants create a person-centered plan that reflects their unique needs and goals.
What Stays the Same?
● Providers and Caregivers: You can keep your current caregivers and continue working with Lifeworks.
● Eligibility: If you were eligible for PCA or CSG, you qualify for CFSS.
● Self-Direction: You maintain control over your care.

● Consultation Services: Work with a consultation services provider to create your CFSS service plan. Lifeworks can guide you through this process.
● Budget Model: Lifeworks offers CFSS through the budget model, giving you flexibility in managing your care and finances.

CFSS Eligibility
To qualify for Community First Services and Supports (CFSS), individuals must complete a MnCHOICES Assessment, live in the community, and be enrolled in the Medical Assistance (MA) or Alternative Care programs.
If you currently receive Personal Care Assistance (PCA) or Consumer Support Grant (CSG) services, your eligibility for CFSS will remain the same.
